Referral Forms: Register with Carer Gateway

Care2Serve, the Tasmanian provider of Carer Gateway, delivers a range of free support and services to carers caring for a family member or friend with disability, mental ill health, chronic or life-limiting condition, alcohol or other drug dependence or who is frail or aged.

By completing one of the forms below you can register yourself or refer someone to be registered with Carer Gateway.

All information that you provide is stored and managed according to our Privacy and Consent Policy, which is based on Australian Privacy Principles and Standards.

By sharing your personal information, you consent to its collection and storage in our database. As part of our funding, we must report information to the government about who we support. This may include information about you, but you will never be identified in the reports. The information helps the government and us to improve services for carers.

Referral Options

Short Referral Form

This short form will take 2-3 minutes to complete. You will be asked to select your referral type:

  • Self-referral (if you are a carer)
  • Referral for someone you know (if you know someone who is a carer)
  • Organisation referral (if you are an organisation or a professional who would like to refer a carer)

Long Referral Form

This comprehensive form takes 5-10 minutes to complete. It helps us gather more detailed information, reducing the need for follow-up calls. This form is especially useful for carers who prefer to provide more information online.
