Module 5 · Planning for Life and Preparing for End of Life

Welcome

Welcome to the fifth module on supporting our elders ‘Planning Life and Preparing for End of Life‘.

Planning is important for carers and the person you care for but sometimes it’s hard to know where to start.

These modules are designed to help carers. A carer is a person who provides unpaid practical and emotional support to someone with a disability, chronic or life-limiting illness, mental ill health, has alcohol or other drug dependence or who is frail or aged.

Carer Gateway/Care2Serve are here to support carers, offering free access to practical advice and support.

Call Carer Gateway and register as a carer – 1800 422 737 (press 1 at the menu).

We acknowledge the traditional owners of the lands and waters upon which we work, live, and sustain ourselves. This land was never ceded, and we acknowledge that the Tasmanian Aboriginal people are its continuing custodians. We pay our respects to Elders past and present.

We welcome, support and include carers who may identify as LGBTIQA+, carers with disabilities or from culturally and linguistically diverse backgrounds.

We value and respect the diversity of carers, their lived and living experiences, and recognise that carers are the experts in their own lives.

Module 5 · About 20 minutes

Planning for Life and Preparing for End of Life

Being a carer can be especially demanding when it comes to helping arrange all the necessary legal and financial documents.

This module will provide you with the information to help an elder put in place all the documents they need to ensure their wishes are understood, legally recorded and can be carried out. These documents include the Will, Enduring Power of Attorney, Enduring Guardianship and Advanced Care Directive.

This module will also alert you to preventative measures to help protect your elder from abuse and scams.

This module will take about 20 minutes to complete.
